WASHINGTON, Aug. 22 (TNSoth) -- The New York State Department of Health has reported an incident to the Nuclear Regulatory Commission involving a radiography source that became stuck in its unshielded position. The event occurred on August 12, 2025, at Aegeus Inspection Solutions Inc., Houston, Texas, when a 73.9 Ci iridium-192 source in a Sentinal 880 Delta device failed to retract.
